DIRECT CARE MODEL

Direct Care is reshaping healthcare by offering a direct, no‑hassle connection to your provider through an affordable monthly membership. By cutting out insurance intermediaries, we streamline care, freeing up more time and resources to focus on what truly matters—your health. Freed from the burdens of endless paperwork and red tape, Dr. Coe, a proud member of the LGBTQ community, offers compassionate, personalized care that’s specifically attuned to the unique needs of LGBTQ patients and allies alike.

In the traditional healthcare system, providers often juggle thousands of patients, leading to rushed visits and long waits. At PridePoint Health, we take a different approach. With a smaller patient panel, we offer longer appointments, faster scheduling, and truly individualized care. By removing the barriers of insurance, we also help you save with wholesale pricing on labs, while still allowing the flexibility to use your insurance for labs, medications, and specialty care when needed.

Our membership model makes healthcare simpler, transparent, affordable, and focused on what really matters—your well‑being.

A few of the many benefits include:

  • Enhanced access – Longer visits, same‑ or next‑day appointments, and direct access to your doctor when you need it.

  • Cost transparency – No co‑pays or deductibles; clear pricing for labs and meds.

  • Patient‑driven care – A real relationship with a doctor who knows your goals and history.

  • Small, community‑based clinic – Local, LGBTQ‑affirming care that’s personal and consistent.

It means your gender‑affirming care is designed around you, not insurance rules. You get longer visits, same‑ or next‑day scheduling, direct messaging with your physician, and continuity of care. We manage HRT starts and maintenance, order and interpret labs, coordinate refills, and help you navigate side effects—without copays or surprise bills for routine visits.

Because we’re not billing insurance for clinic time, we can focus on thoughtful dose changes, shared decision‑making, and regular monitoring aligned with WPATH standards—so your plan matches your goals and safety needs.

GETTING STARTED & ELIGIBILITY

We know healthcare costs are a major concern. That’s why we chose the direct care model—an affordable alternative that often costs less than your daily coffee or phone bill. Many members actually save money compared to traditional care.

No copays, no surprise inflated bills, and all visits included. Members also get wholesale pricing on labs and medications, same‑ or next‑day appointments, direct access to their doctor, and longer, more personalized visits.

Yes! Pairing our Gender Affirming Care (GAC) membership with your health insurance can lower your total costs while improving access.

Compared to a High Deductible Health Plan alone, adding GAC can save $700+/year. Versus a gold, low‑deductible plan with copays, savings approach $2,000/year. See our full cost comparison.

  • Direct access to a doctor who knows you
  • No waits and unrushed visits
  • Included services
  • Convenient lab testing
  • Wholesale pricing on labs, imaging, and medications
  • Are you frustrated with today’s healthcare system?
  • Do high costs or surprise bills keep you from seeing a doctor?
  • Have you waited weeks for a visit—then been rushed in 10 minutes?
  • Do you want a long‑term relationship with a doctor who knows you and your goals?
  • Would you value same‑ or next‑day appointments that start on time, plus handling simple issues over email, text, or phone with your physician?

If you answered “yes” to any of these, PridePoint Health could be the right choice—personal, affordable, barrier‑free care when you need it most.

We offer tailored memberships (PrEP & STI care, Gender‑Affirming Care, and full DPC) so you can get exactly what you need without paying for what you don’t. You can combine PrEP + GAC at a discounted rate of $89/month. Our DPC membership includes services from both plans.

To keep things simple and consistent for all patients, we don’t offer one‑off visits outside our membership model.

Yes—minimum 3 months. We invest heavily in intake and onboarding to build a safe, tailored plan; the initial commitment ensures a solid foundation.

You can message your doctor directly via text and phone 24/7. We aim to address needs within 1–2 business days (often sooner).

BILLING & INSURANCE

After your first 3 months, just give us 30 days’ notice. If you prepaid annually, we’ll refund the remaining balance.

Yes. We accept HSA/FSA cards, checks, cash, credit/debit, and ACH. For HSA/FSA specifics, consult your tax professional.

Remember, insurance companies don’t deliver healthcare—clinicians do. Even the best plan can still mean rushed visits, long waits, and surprise bills. We can help change that.

We don’t bill insurance directly, but you can still use it for services outside our care—medications, lab work, imaging, specialist visits. Our membership removes co‑pays/deductibles on routine visits and includes direct access to your physician. Many members save thanks to wholesale pricing on labs/meds. See how our model compares.

Absolutely. We recommend carrying HDHP or health‑sharing coverage for major expenses, but we don’t turn patients away. GAC membership includes as many services as possible and provides 50–90% discounts on labs.

CARE DELIVERY & SERVICES

Yes. Gender‑affirming care is medically necessary. We initiate and continue HRT for members who agree to routine monitoring, follow‑up, and otherwise meet WPATH criteria. Curious how getting started works? Read our overview: What to expect when starting hormones with PridePoint.

Care plans are individualized and may include masculinizing or feminizing hormones (testosterone, estrogen, progesterone), anti‑androgens (spironolactone, finasteride), and adjunct therapies—based on your goals and safety labs.

Often, yes. We’ve provided telemedicine across Oregon and Washington for years. For feminizing regimens (no DEA‑controlled meds), a physical exam from your local clinician is typically sufficient and we handle the rest remotely.

For masculinizing regimens with testosterone (a DEA‑controlled med), policy is evolving. Currently a local exam is sufficient; if rules change, we may require an in‑person exam at intake and annually. We’ll keep it seamless.

We coordinate labs near {{city}} (or elsewhere in {{region}}/{{state}}), and send prescriptions via mail order or your local pharmacy.

We provide gender‑affirming care statewide in {{state}} via telehealth, including the {{city}} area and the broader {{region}}. In‑person visits are available at our Portland clinic.

  • Local & independent. We’re a small, Portland‑based clinic embedded in the LGBTQ community—not a national platform. You’ll know your doctor.
  • In‑person option. Telehealth statewide plus face‑to‑face visits at our clinic when you want or need them.
  • Continuity with a board‑certified physician. You won’t be passed from provider to provider; your care is led by a physician with years in gender specialty settings.
  • Standards‑based. We follow WPATH‑aligned practices with thoughtful monitoring, dose adjustments, and safety labs.
  • Integrated care. Need primary care or sexual health support too? Our memberships fit together—no fragmentation.
  • Transparent pricing. No copays for routine visits; wholesale rates for labs/meds.

Generally, no. We assess readiness in‑house with a thorough history and physical. We coordinate with other clinicians only when it clearly benefits the patient.

Clinic blood draws are included; the processing costs are not. You can use insurance, or take advantage of our negotiated wholesale lab pricing (often 80–90% off retail).

Yes—wholesale pricing on hormone meds and labs. You pay our cost + a small 10% fee. Often cheaper than insurance.

  • Estradiol tablets: ~ $6 for 90 (vs. $30+ retail)
  • Feminizing labs: $12–$14 (vs. $100+)
  • Masculinizing labs: ~ $10 (vs. $90+)

We can dispense many meds in‑clinic or arrange discreet mail‑order. Note: We can’t dispense controlled substances (e.g., testosterone) from clinic.

Dr. Coe is an internal medicine physician who cares for adults; we do not treat patients under 18.

When gender-affirming hormones do not fully stop menstruation, or when there is ongoing pregnancy risk, we can provide select hormonal contraceptive options tailored to your HRT regimen. We also treat sexual dysfunction and side effects related to HRT. These services are included in your membership.

For trauma-informed, gender-affirming reproductive procedures—such as pap smears, IUD or Nexplanon placement, STI assessment and treatment, and other genital care—we recommend joining our full DPC membership. Otherwise, these additional services should be accessed through your primary care provider.

Our maintenance membership helps lower costs for patients who are stable on hormones and only need once‑yearly visits with ongoing prescription management. Eligibility requires:

  • 12+ consecutive months of routine gender‑affirming hormones
  • Established PridePoint member for 6+ consecutive months
  • No dose/formulation changes for 6+ consecutive months
  • Last 2 hormone labs in safe range over at least 6 months
  • No active need for our involvement in prior/future GA procedures/surgeries
  • Agreement to annual lab monitoring and annual visit (virtual or in‑person)
  • Agreement to return to regular GAC membership if eligibility changes

Ongoing eligibility: We reassess these criteria over time. If anything changes—for example, unsafe or supra‑therapeutic levels, a needed medication adjustment, or plans for surgery—you’ll shift back to the regular GAC membership until you meet the requirements again. This isn’t a penalty; it simply ensures you’re getting the right level of support as your needs evolve, while keeping care as affordable as possible.

The maintenance plan includes annual lab monitoring and a yearly visit, plus ongoing prescription refill management.
