DIRECT CARE MODEL
-
Direct Care is reshaping healthcare by offering a direct, no-hassle connection to your provider through an affordable monthly membership. By cutting out insurance intermediaries, we streamline care, freeing up more time and resources to focus on what truly matters—your health. Freed from the burdens of endless paperwork and red tape, Dr. Coe, a proud member of the LGBTQ community, offers compassionate, personalized care that’s specifically attuned to the unique needs of LGBTQ patients and allies alike.
In the traditional healthcare system, providers often juggle thousands of patients, leading to rushed visits and long waits. At PridePoint Health, we take a different approach. With a smaller patient panel, we offer longer appointments, faster scheduling, and truly individualized care. By removing the barriers of insurance, we also help you save with wholesale pricing on labs, while still allowing the flexibility to use your insurance for labs, medications, and specialty care when needed.
Our membership model makes healthcare simpler, transparent, affordable, and focused on what really matters—your well-being.
-
A few of the many benefits include:
Enhanced access – Say goodbye to long waits and rushed appointments. With direct care, you get longer visits, same or next-day appointments, and direct access to your doctor when you need it.
Cost transparency – No more surprise bills or confusing medical costs. With direct care, you know exactly what you’re paying, with no co-pays or deductibles. You won’t need to shell out thousands to meet your deductible just to access high-quality, all-inclusive care.
Patient-driven care – Experience healthcare where your doctor can truly focus on you. Build a real relationship with a doctor who knows you and your health goals. With no insurance barriers or job changes to worry about, we’re here to ensure you always have access to the care and medications you need—because we answer to you, not insurance companies.
Small, community-based clinic - When you choose us, you’re not just getting excellent care—you’re supporting a small, local LGBTQ business that truly cares about its community. In a healthcare world dominated by big corporations where decision-makers are far removed from patient care, we stay personal, compassionate, and community-focused.
GENERAL & MEMBERSHIPS
-
We know healthcare costs are a major concern. In the corporate healthcare system, multiple stakeholders drive up expenses, leaving patients with higher bills and less time with their doctor. That’s why we chose the direct care model—an affordable alternative that often costs less than your daily coffee or phone bill. Many of our patients actually save money compared to traditional care.
With no copays, no surprise inflated bills, and all visits included, you’ll know exactly what you’re paying for. Plus, our members enjoy wholesale pricing on labs and medications, same- or next-day appointments, direct access to their doctor, and longer, more personalized visits. Our mission is simple: make high-quality care both accessible and affordable—so you can focus on your health, not your medical bills.
-
Yes! Pairing our Gender Affirming Care (GAC) membership with your health insurance can lower your total healthcare costs while giving you better access to care.
Compared to having a High Deductible Health Plan (HDHP) alone, adding GAC can save you over $700 per year. And when compared to a gold, low-deductible plan with copays, the savings approach $2,000 per year. See our full cost comparison.
And remember — all visits are included in your monthly membership, with no copays or surprise bills. You’ll also enjoy:
Direct access to a doctor who knows you
No wait times and unrushed visits
A long list of included services
Convenient lab testing
Wholesale pricing on labs, imaging, and medications
It’s not just about spending less — it’s about getting more from your healthcare.
-
Do you feel frustrated with the current healthcare system? Are you hesitant to see a doctor because of the high cost? Have you ever been hit with a surprise bill weeks after a visit? Do you want a long-term, meaningful relationship with your doctor? Do you feel like there are too many barriers between you and the care you need? Would you appreciate same-day or next-day appointments that actually start on time? How about handling simple issues over email, text, or phone with your doctor?
If any of this resonates with you, PridePoint Health could be a great fit.

We offer different memberships to provide tailored care for specific needs, such as PrEP and STI care or gender-affirming care. We recognize the distress of not knowing if your provider is experienced or even comfortable managing gender affirming hormones or discussing sexual health in a non-judgmental way. These memberships allow us to deliver essential services at a lower cost for those who don’t need comprehensive primary care. This also ensures continuity and personalized care without interruptions to your prescriptions. We offer a discounted rate of $89/month for those that wish to combine both PrEP and Gender Affirming memberships.
For those seeking full primary care, our DPC membership includes all services from the PrEP and Gender Affirming Care plans.
-
To keep things simple and consistent for all of our patients, we do not provide visits or care outside of our standard membership system.
-
Yes, there is a minimum commitment of 3 months. Our goal is to build long-term, trusting relationships with our patients. To ensure we provide the highest quality care, we invest significant time and resources in the intake and onboarding process. This initial commitment allows us to get to know you and your healthcare needs, ensuring a solid foundation for personalized care moving forward.
-
We strive to provide our patients with unprecedented access to their doctor. You can message your doctor directly via text and phone 24/7, and we’ll do our best to address your needs within 1-2 business days—no more waiting days on end to hear back from your doctor!
BILLING & INSURANCE
-
If you decide to cancel after your first 3 months, just give us a 30-day notice—no hassle. If you’ve signed up for an annual term and choose to cancel, we’ll simply refund the remaining balance of your membership. We’re always here to make the process smooth and straightforward for you!
-
HSA and FSA debit and credit cards are accepted. We take check, cash, credit card, debit cards and ACH. For more specifics on HSA/FSA funds, please consult your accountant.
-
While we don’t bill insurance directly, you can still use your insurance for healthcare outside of our services, such as medications, lab work, imaging, and specialist referrals. Our membership model is designed to simplify your experience, eliminating co-pays, deductibles, and surprise bills, while providing you with personalized, comprehensive care. Many patients find that our all-inclusive care, coupled with wholesale pricing on labs, helps them save money—even with great insurance.
-
Absolutely. Although we recommend everyone carry high deductible insurance or health sharing coverage, we do not turn anyone away due to lack of insurance. PridePoint Health does its best to make comprehensive care affordable for those with or without insurance. We pack a lot of value into your membership. Our Gender Affirming Care membership includes as many services as possible in your plan, and you get wholesale pricing on labs with discounts ranging from 50-90% off.
GENDER AFFIRMING CARE
-
We are here to support you and understand that gender-affirming care is a medically necessary and essential service. We initiate and continue management of HRT for all members who agree to routine monitoring, follow up, and otherwise meet WPATH criteria.
-
Gender exists on a spectrum, and your care plan will reflect your unique goals and identity. We work closely with you to understand how you want to express yourself and tailor a plan to help you achieve your goals. This may include masculinizing or feminizing hormones (Testosterone, Estrogen, and Progesterone), anti-androgens (Spironolactone and Finasteride), and a variety of other medications and therapies to help you feel and look your best.
-
Yes, in many cases, gender-affirming care can be provided entirely through telemedicine. We have years of experience offering telemedicine services throughout Oregon and Washington. For feminizing hormone regimens, which do not involve DEA-regulated prescriptions, a physical exam from your local primary care clinician is typically sufficient, and we can manage the rest of your care remotely.
For masculinizing hormone regimens that include testosterone (a DEA-regulated prescription), telemedicine laws are currently in flux. At this time, a physical exam with your local primary care clinician is adequate. However, we are monitoring the situation closely, and if laws change, we may need to require an in-person physical at intake and annually thereafter. Rest assured, we will work with you to ensure the process is as seamless as possible.
Schedule a free meet and greet with Dr. Coe discuss your options and tailor a membership that best fits your needs.
-
Generally, no. The evaluation for readiness to start HRT is done by Dr. Coe in-house with a thorough history and physical. However, there are some circumstances where coordination with additional healthcare or mental healthcare providers may be necessary. This is only pursued when it serves the best interest of the patient.
-
While lab draws performed in our private clinic are included at no cost, the costs for running the lab tests are not included in the membership. Some patients prefer to use their insurance to cover the cost of lab testing. For those with lab co-pays or those who are uninsured or underinsured, we negotiate wholesale lab pricing at 80-90% discounts, often making lab work more affordable than it would be if using insurance. Our goal is to provide cost-effective options for all patients.
-
Yes — our gender-affirming care members get access to wholesale pricing on both hormone therapy medications and lab testing. You pay what we pay plus a small 10% fee to cover our costs — no hidden charges — and it’s often less than running them through insurance.
For example:
Estradiol tablets: Around $6 for 90 tablets — compared to $30 or more at retail pharmacies.
Feminizing hormone panel: $12–$14 — compared to $100 or more in the traditional model.
Masculinizing hormone panel: $10 — compared to $90 or more in the traditional model.
Many members save hundreds of dollars a year by using our wholesale pricing for medications and labs. We can dispense medications directly in clinic or arrange discreet mail-order delivery for your convenience.
Note: We’re unable to dispense controlled substances (e.g. testosterone) through our clinic.
-
Dr. Coe is an internal medicine doctor, which means his training and expertise are focused on adults. He is unable to provide medical care for folks under age 18.
-
In cases where gender affirming hormones do not effectively stop menstruation, or when there is ongoing pregnancy risk, we offer select hormonal contraceptive options, tailored to the specific HRT regimen. We also treat sexual dysfunction and side effects of HRT. This is all included in the membership.
For those needing additional trauma-informed, gender-affirming reproductive health care, such as pap smears, IUD/Nexplanon implants, STI assessment and treatment, and other genital conditions, we recommend joining our full DPC membership. Otherwise, these additional services should be accessed through your primary care provider.
-
We offer a maintenance membership specifically designed for gender affirming care to help lower costs for patients who are stable on hormones and only require ongoing management of their maintenance prescription with once yearly visits. To join the maintenance membership, you must meet the following requirements:
Routinely taking gender affirming hormones for at least 12 consecutive months.
Be an established member of PridePoint Health for at least 6 consecutive months.
Have had no changes in your hormone dose or formulation for at least 6 consecutive months.
Last 2 hormone lab levels in safe range, over a period of at least 6 months.
Not requiring our medical expertise or involvement in prior or future gender affirming procedures or surgeries.
Agree to annual lab monitoring and annual virtual or in-person visit.
Agree to return to regular Gender Affirming Care membership if no longer meeting eligibility requirements.
Eligibility is assessed on an ongoing basis. If at any point you no longer meet the eligibility criteria, for example, due to unsafe or supra-therapeutic hormone levels, needing a medication adjustment or deciding to pursue surgery, etc., you agree to return to the regular membership until you meet the eligibility requirements again. This ensures we can continue to offer the appropriate level of care as your needs evolve, while still making this care as affordable as possible for our patients. The maintenance membership is priced for both virtual and in-person care, providing flexibility and continued support. Membership includes annual lab monitoring and a visit, as well as ongoing prescription refill management.
