How to Request and Share Your Health Records

Keeping your health records up to date allows us to provide you with the best care. Whether you’re transferring records from a previous provider or sharing records from a recent visit with a specialist or outside clinic, there are several ways to request and upload your records.


How to Check if Your Records Are in MyChart

Many healthcare systems use Epic’s MyChart, making it easy to access and share records. Here’s how to check:

Search for Your Provider’s Patient Portal:

  • Go to your previous provider’s website and look for their patient portal section. If it mentions MyChart, you likely have access to your records there.

  • Some organizations have their own branded version of MyChart (e.g., MyPeaceHealth, Providence MyChart).

Try Logging into MyChart:

  • If you’ve used MyChart before, log in at www.mychart.org or open the MyChart app and search for your provider.

  • If your provider appears in the search, they use MyChart, and your records should be available there.

Instantly Download Your Records:

  • Once logged in, access the Menu options, then select ‘Document Center’ under the ‘My Records’ section to request your records for download.

Requesting Records from Providers Who Use Other Record Systems

If your prior provider doesn’t use MyChart, here’s how to get your records:

1. Check If You Can Download Your Records

  • Many electronic medical record (EMR) systems allow patients to download their records directly from the patient portal.

    Log into your previous provider’s patient portal and look for:

    • “Download Medical Records”

    • “Export Records”

    • “Download/Share My Record”

Select PDF format whenever possible—this makes uploading to our portal much easier.

2. Contact the Provider’s Office

  • If you can’t download records online, call your provider’s Medical Records Department (Health Information Management, or HIM) and ask how to request a copy.

  • Some providers allow requests through their patient portal, email, or by phone.

  • Be sure to request records electronically in PDF format for easy upload.

3. Use Our Records Request Form

  • If your provider requires a signed request form or doesn’t provide one, you can use our records request form to authorize them to send your records directly to us.

4. Submit a Records Release Form from Your Provider

Some providers require you to complete a Medical Records Release Form (Authorization to Release Health Information) before they can share your records.

  • The form usually asks for:

    • Your name, date of birth, and contact information

    • The provider or facility that holds your records

    • The specific dates or types of records you need (e.g., visit notes, lab results, imaging)

    • Where to send the records (request electronic or PDF format whenever possible)

📌 Under federal law, providers must release records within 30 days.
